Certification guidelines require electronic health record (EHR) technology to provide a user the ability to document a patient’s request for an amendment to their electronic health information.
Instructions
- Access Patients > search for and open a patient > Amendments.
- Click the + icon.

Complete the required fields.
- Status: If the status of Submitted is used, we recommend using the Tasks module to create a reminder to verify submitted information and return to change the status to Accepted or Denied.
- Click 'Save.'
- The action of Save from step #4 changes the screen view to include a Documents & Images section. Click 'Upload' or 'Scan' to attach documentation to the Amendment.
- Click 'Save.'
