The following instructions are to add a consent form template within the Admin module.
Instructions
- Access Admin > Documents and Forms > Consent Forms.
- Click the + icon.
- Complete the required fields.
- Click 'Save.'
- Click 'Upload.' Note: This button does not display until after Save has been used.
- Select the document from your computer and click 'Open.'
- Click 'Save.'
