Remember: If you are required to report, in order to avoid a negative payment adjustment in 2027, your 2025 MIPS overall score must meet or exceed the 75 points performance threshold for 2025 MIPS. Scores above 75 may result in a positive payment adjustment.

Reminder: Preliminary Scoring No Longer Available During Submission
As a reminder, CMS eliminated the preliminary score and preliminary category-level scores from submission beginning with data submission for the 2023 performance year. You’ll be able to preview your 2025 final score in mid-June 2026 and view your 2027 MIPS payment adjustment information in mid-August 2026.
Getting started:
The process of MIPS data submission to Medicare can be summarized in a few steps:
1. Check and understand your eligibility on the Quality Payment Program website.
Entering your NPI in the participation status tool allows you to understand if Medicare is requiring you to submit data and, if not, if you have a path to opt-in to the program and be considered from Medicare in the future.
2. Register for Quality Payment Program (QPP) portal access.
A username and password are needed to access the Quality Payment Program portal for your practice. If you have not established them before or haven’t used them in a while, you’ll need to re-register. This can be done by following the instructions for CMS’ HARP system. Please note that the preceding link will initiate a zip file download to your computer containing series guidance documents.
3. Use the MIPS – PI (Stage 3) scorecard to analyze and collect 180 days of data.
*Note: Clinicians in small practices will have their MIPS - PI category score automatically reweighted and do not need to submit data for this category.
