The importance of signing encounters can be easy to overlook but the signing process should always be completed. A doctor’s signature at the conclusion of an encounter is their attestation to the truthfulness and accuracy of the medical record. In other words, it is the doctor’s word that what is in the chart actually took place. Without that verification in place, a third party is provided room to argue that the encounter never took place and recoup payment in an audit. Thus, it is very important to sign your encounters when your documentation has been completed.
Providers can sign an encounter in two ways:
- The sign encounter icon located beside the provider's name in the header of the encounter.
- The Sign Encounter button located on the Coding screen.

Instructions
Sign Encounter Icon
- Open an encounter.
- Click the sign encounter icon. Note: As the icon is clicked to sign the encounter, the system navigates to the Coding screen to ensure all data within the encounter is saved, and a Sign Encounter modal displays.
- In the Sign Encounter modal, input your RevolutionEHR password.
- Click 'Save.' Note: The icon color changes from orange to green to indicate the encounter is signed.

- Click 'Sign Encounter.'
- In the Sign Encounter modal, input your RevolutionEHR password.

Helpful Information

- The sign encounter icon and the Sign Encounter button do not display in the following situations:
- The encounter does not have a Coding screen in the workflow steps.
- The user is not a provider.
- The user is not assigned as the provider on the encounter.
