If you receive an error message when you have attempted to print or preview a CMS-1500 form that includes diagnoses in excess of twelve, the issue only affects the print and preview of the claim form, it does not affect the sending of the electronic claim 837 file.
The electronic CMS-1500 form allows diagnoses in excess of twelve, however, the printed form does not allow diagnoses in excess of twelve. If there are thirteen or more diagnoses and you want to print or preview the claim, you must adjust the diagnoses to twelve or less.
