The Claim Frequency Code is the same as the Resubmission Code, which is Box 22 on the CMS form.
Box 22 Resubmission Code is located on the insurance invoice, within the Additional Claim Info tab. Your invoice must be pending if you need to edit it.
The value entered in Box 22 is being rejected. Possible values that can be used are:
1-original
6-corrected
7-replacement
8-void
Each insurance company has their set of rules for usage. e.g., Medicare - only 1-Original is valid. Contact your insurance company for the correct usage. Also, since you are contacting the insurance company, verify the number that should be entered in the Original Reference # field. When 6, 7, or 8 is used, the Original Reference # is required. Unfortunately, we are unable to provide this information.
Possible values for the Original Reference number are the Claim ID, the ICN (internal control number), or the DCN (document control number).
After you have the correct Claim Frequency Code and Original Reference #, we recommend that you enter this information in the insurance company record's Notes section. Each insurance company can have difference usage values. See Example
